Appointment Scheduling

When you arrive, you will be asked to complete a history to give the doctor adequate information about your health. Your referring physician can send records to us. Please bring a list of all medications you are currently taking, including over-the-counter medications and supplements.

Due to the nature of high risk pregnancies we do not allow children in our office.

Fees & Billing

All copays, co-insurance and deductibles are due at the time services are rendered.

We ask that you provide your current insurance information at every visit, to make sure we have your most up to date insurance information.

Insurance

Our office accepts most insurance plans. Our staff will be happy to file the insurance for you. Your insurance coverage is an agreement between you and the insurance provider. However, the final responsibility for payment of the bill belongs to you..
